Nominations

  Our committee would like to encourage members of the community to send in nominations for the 2020 PHS Athletic Hall of Fame!  Please note - We are only accepting nominations from active years 1966-1999.
Steps to Nominate:

1. The form below must be completed and sent in to the committee.  You have two options to send in the completed form and any supplemental material.  Please choose one of these two options:
